Data Analysis Leads to Better Treatment For Frequent Users of Social Services

The Familiar Faces Initiative Leadership Network visited Johnson and Douglas Counties, Kan. on April 11-13 to learn about how these counties partner in using My Resource Connection, a cross-system data-sharing system that matches mutual clients across providers to target familiar faces in their communities and provide outreach, as well as integrate and coordinate care.
