Mission

Through the Familiar Faces Initiative, the National Association of Counties empowers communities to share data and integrate care options between health and justice systems so they can intervene earlier, improve outcomes and reduce incarceration and hospitalization rates.

Advisory Board

The Familiar Faces Advisory Board is comprised of key organizations and associations across multiple sectors and spheres of influence to create sustainable and impactful change to local policy and practice. These organizations have expertise or provide a key service relating to familiar faces.

Founding Network

The Familiar Faces Initiative began in 2016 as the Data-Driven Justice initiative. Over the first five years of the initiative, 151 communities joined, committing to sharing data across systems to disrupt the cycle of incarceration and crisis for familiar faces. The list below reflects the original 151 communities and the network continues to grow today.
